She's suffering from internal bruising of the liver and kidneys.
The doctor pulled me to one side and asked me to do 'half the job' in future to save the poor girl from further damage.

Immediately she'd been examined, the doctor came and asked to examine me, pretty thing she was so when I dropped my strides I had half a lob on. She looked me straight in the eye and told me to half the dosage or risk damaging the Duchess permanently.
